Dosing and safety
TriMix is self-administered as a small injection 15–30 minutes before sexual activity. It should not be used more than once in a 24-hour period or more than three times per week. Your provider will start you on a conservative dose and gradually adjust it based on your response. We provide detailed injection instructions, ultra-fine needles, and all necessary supplies. Each 5 mL vial contains up to 10 doses at the maximum prescribed dose of 0.50 mL. Because treatment begins at a low dose and is gradually titrated, many patients will have medication remaining at the end of the month. To help maintain sterility, any unused medication should be discarded 28 days after the vial is first punctured, even if medication remains.

The most serious risk is priapism — an erection lasting longer than 4 hours. This is a medical emergency that requires immediate care. Refills run every 25 days on monthly plans and every 85 days on quarterly, timed so your next supply arrives before you run out. One note for injectables like TriMix: per FDA/USP guidelines, vials have a 28-day beyond-use date once punctured. Our plans have been priced to ensure you never have to choose between safety and affordability.

Standard dosing protocol — your prescriber may adjust this:
Inject 0.05–0.10 mL at the base of the penis before sexual activity. Increase by 0.025–0.05 mL based on response. Wait at least 48 hours before repeating or increasing the dose. Maximum 0.50 mL per dose; no more than 3 doses per week.
Our standard formulation is T-105 TriMix (papaverine 30 mg / phentolamine 1 mg / PGE1 10 mcg per mL), and it’s the only one we prescribe for new patients. T-105 is a well-established, balanced formula that works for the vast majority of men, and standardizing on it lets our prescribers fine-tune your results through dosing adjustments rather than switching formulas. It also means our pharmacy compounds it at higher volume, which keeps quality consistent and pricing lower.
Super TriMix (papaverine 30 mg / phentolamine 2 mg / PGE1 40 mcg per mL) is available on special request only. It contains four times the alprostadil (PGE1) and twice the phentolamine of T-105, and it’s priced at $249 per 5 mL vial. It is not an option for first-time patients — your prescriber will only consider it after you’ve titrated through T-105 per protocol without an adequate response. Super TriMix is also not available for shipment to Alabama, Arkansas, California, Hawaii, Kansas, Michigan, Minnesota, or Mississippi.
We do not offer BiMix, QuadMix, or T-106.
